REQUEST SCHOOL FORMS

There are two sets of form requests to choose from: Set A and Set B.

Use these forms to request Transcripts, Driver's License forms, Dual Enrollment, Verification Letters, and other documents.

› Forms

A Set A Forms:

The first group includes those that we regularly issue at no charge.

B Set B Forms:

The second group has forms that require a processing fee.

Once you select the form you need, you will be directed to a PayPal page where you can make secure credit card or debit card payments.

You do not need a PayPal account to complete the process.

Note: Students currently enrolled refers to members who are enrolled for the 2023-24 school year that ends in June 2024.

1 Dual Enrollment w/ College

These forms are issued by the colleges, and need to be brought to our office to complete. No charge.

2 Social Security Verification

These forms are issued by the social security office, and need to be brought to our office to complete. $15 fee.

SET A FORMS

A Driver's Permit Form

Do NOT visit the Motor Vehicles Office without this form!

1) First request. No Charge.
(Please use SET A FORMS).

2) Additional copies incur $10 fee.
(Please use SET B FORMS).

Requirements by the Tennessee Department of Motor Vehicles:

»	The student is enrolled for the current school year.
»	New students have been enrolled at least 30 days in the current school year.
»	A copy of the student's birth certificate and social security number is currently in our files.
»	The student's semester reports and curriculum lists are up-to-date.

B Unofficial Transcript

First request. Current high school students only. No Charge.

C Enrollment Verification

Student currently enrolled. No Charge.

D Work Letter

Student currently enrolled. No Charge.

E School ID Cards

First request. No Charge.

F Sports Participation Letter

No Charge.

1 Official Transcript

For both current and previous students. $15.

2 Transcript: Good Status

For insurance. Current students only. $15.

3 Driver's Permit Form

Additional copy after first request. $10.

4 Semester Reports

Copies of submitted semester reports with test scores. $15.

5 Student's File

Copies of the student's current school files. $30.

6 High School Diploma

When requested for other / separate graduation ceremony. $30.

7 Diploma Duplicate

Replacement copy for previously issued diploma. $30.

8 Diploma with Cover

When requested for other / separate graduation ceremony. $60.

9 ID Cards Replacement

Additional School ID Cards. Current students only. $10.
